This is an anonymous survey with questions designed to get a quick-check on the “web pulse” for this course. Your evaluation will help to improve the course design and instruction. The requested reply date is <insert reply date>. You will receive points for completing this activity because the Gradebook shows who has and who has not completed the survey. However, your identity will not be revealed. In other words, the report of your replies will contain all of the replies without individual names or any other identifying information.
